How to Apply for Ayushman Bharat Card 2026

  • Get your documents ready: Aadhaar card of all family members you want to add, ration card if available, mobile number linked with Aadhaar, and a passport size photo. For 70+ senior citizens, age proof like Aadhaar or voter ID is sufficient.
  • Visit the nearest Common Service Centre (CSC) or the Ayushman Mitra desk at any empanelled hospital. You can find the nearest empanelled hospital on pmjay.gov.in by entering your district.
  • At the CSC or hospital, the operator will verify your eligibility on the PM-JAY portal using your Aadhaar or mobile number. They will do biometric authentication using your fingerprint or iris scan.
  • After verification, your family details will be entered into the system. The operator will click your photograph and upload the family data. The Ayushman Card will be printed on the spot or within 24 hours.
  • The card activates within 24 to 48 hours. You will receive an SMS on your registered mobile number when the card is active. Keep the card safely - you need it for every hospitalisation.
  • To use the card, visit any empanelled hospital across India with your Ayushman Card and Aadhaar card. The hospital Ayushman Mitra will verify your identity, and if the treatment is covered, you will be admitted for free cashless treatment. No money to be paid to the hospital.
